I understand this study aims to research the socioeconomic status, education requirements, licensing requirements, labor conditions, job benefits, education desires, and personal perspectives on being a cosmetologist. *

I understand that we are asking you if you want to be in this study because you are a licensed cosmetologist in the United States.*

I understand that this study is being conducted by Margaret Ferguson, Indiana University, and Beyond the Chair Co. It is funded by the Indiana University Advanced Summer Research Scholarship.*

I understand that I am agreeing to participate in the study, and I will: read and answer each question on the survey as honestly and accurately as possible, which will take upward of 10-25 minutes.*

I understand that in agreeing to participate, I have considered the risks and the potential benefits of taking part in this study.*

I understand that I may be uncomfortable while answering the survey questions. While completing the survey, I can skip any questions that make me feel uncomfortable or that I do not want to answer.*

I understand I will not be paid to participate in this study.*

I understand that Beyond the Chair will protect my information and make every effort to keep my personal information private, but they cannot promise complete confidentiality. No information that could identify you will be shared in publications about this study.*
